Chronic Disease Self Management Program
The mission of the Chronic Disease Self Management Program is to help those 55 and older who suffer from chronic diseases (heart disease, arthritis, diabetes, asthma, bronchitis, emphysema, cancer and others) to move toward self sufficiency. The program gives people the skills to coordinate all the things needed to manage their health and helps them keep active.The program is designed to enhance regular treatment and disease-specific education such as cardiac rehabilitation or diabetes instruction.
Subjects covered include:
- Techniques to deal with problems such as frustration, fatigue, pain and isolation
- Appropriate exercise for maintaining and improving strength, flexibility and endurance
- Appropriate use of medications
- Communicating effectively with family. friends, and health professionals
- Nutrition
- Making informed treatment decisions
