Pathways:

If you live in Pierce County, have Apple Health/ Medicaid, and are under 65 years old – you could qualify for our free Pathways care coordination assistance! Contact us at cbh@kwacares.org or 425-591-2331 to find out more.

What: One-to-one assistance with appointments to medical and behavioral health, chronic health education, benefits enrollment and navigation, assistance applying for housing/ employment/ education programs, and assistance in setting and meeting healthy goals. KWA also provides transportation once a month to a medical appointment.

Who: Those under 65 years old in Pierce County – who have either multiple chronic health conditions, one chronic health and one mental health/substance use condition, or are pregnant.

Health Home with Coordinated Care:

KWA has a contract with Coordinated Care to provide free comprehensive care coordination assistance for eligible adults and children, who have Apple Health insurance through Coordinated Care.

What: One-to-one assistance with appointments to medical and behavioral health, chronic health education, benefits enrollment and navigation, assistance applying for housing/ employment/ education programs, and assistance in setting and meeting healthy goals. KWA also provides transportation once a month to a medical appointment.

Who: Those under 65 years old in Pierce, Snohomish, and Kitsap Counties with Medicaid/ Apple Health – who have severe diagnosed chronic health conditions and frequent use of health systems. Eligibility is determined by the WA Health Care Authority.

Health Home with Pierce County Human Services:

KWA has a contract with Pierce County to provide free comprehensive care coordination assistance for eligible seniors and those with disabilities, who have Medicaid and Basic Medicare Part.

What: One-to-one assistance with appointments to medical and behavioral health, chronic health education, benefits enrollment and navigation, assistance applying for housing/ employment/ education programs, and assistance in setting and meeting healthy goals. KWA also provides transportation once a month to a medical appointment.

Who: Those over 65 years old or with disabilities in Pierce County with dual Medicaid/ Apple Health and

Basic Medicare Part A & B – who have severe diagnosed chronic health conditions and frequent use of health systems. Eligibility is determined by the WA Aging & Long-Term Support Administration e-program.
